A Choice of Care Plans
As a member of Central Coast Direct Medical Care, you can select the level of medical services you would like to receive.
All members, regardless of the level selected, are entitled to the following guaranteed access services:
A person to answer your phone calls during regular business hours,
Prompt return calls for messages left after hours,
Same-day or next-day telemedicine consultation when needed,
Experienced, professional medical advice from board-certified clinicians,
Diagnosis and management of acute and chronic medical problems,
Coordination of diagnostic testing at local laboratory and radiology centers,
Ready access to clinicians by email or telephone,
Rapid reporting of test results,
Prompt specialty referrals,
Expedited insurance authorizations, and
Quick electronic prescribing with timely refills.
Home Care Plans
Membership fees are based on the type of plan chosen, the number of participants residing at a single address, the frequency of payment, and the mode of payment. For the Superior and Elite Plans, there is a discount of $25 per month per person when two or more participants reside at the same address--a savings of $600 per year per household!
Membership fees paid annually by other-than-credit/debit cards receive a 5% discount.
Access Care
Designed for generally healthy adults with fewer medical needs, Access Care enrollees receive our guaranteed access services, and:
One home visit per year
Additional home visits as necessary
Prompt telemedicine visits for acute problems
Routine health screening
Access Care 2025 Fee Schedule:
$100 per month; or $1140 per year when paid annually

Superior Care
Recommended for patients with Medicare insurance or with chronic medical conditions, Superior Care enrollees receive our guaranteed access services, all provisions of Access Care, and:
Expedited doctor or nurse practitioner consultations
Home visits 2-4 times per year for maintenance of chronic medical problems
Liberal use of home health nursing, physical therapy, occupational therapy, speech therapy, medical social worker, wound care, and other ancillary services as needed to speed recovery
Timely specialty referral for complicated medical problems
Review of specialist assessments and recommendations
Regular cardiovascular risk assessments
Annual wellness surveys
Standard vaccine recommendations
Periodic depression screening
Rapid resumption of care after hospital discharge or emergency room visits
Superior Care 2025 Fee Schedule:
$175 per month for individuals; or $1995 per year when paid annually
$150 each per month for multiples; $1710 each per year when paid annually
Elite Care
Elite Care members receive a premium level of primary care services and in-depth supervision delivered to them in the home. Elite Care enrollees receive our guaranteed access services, all provisions of Access and Superior Care, and:
First priority scheduling for home visits
Frequent home visits for maintenance care
Unlimited additional visits for acute problems
Increased time with a health provider to assess all concerns
Unlimited telemedicine visits for new or ongoing health issues
Disease prevention counseling
Chronic care management--continuous oversight of chronic medical problems
Seamless transition of clinical oversight within hours of hospital discharge or emergency room care to ensure continuous monitoring and recovery
Advanced care planning--discussion and preparation for your future medical care
Expedited specialty referral--including university medical centers when necessary—for a broader team approach to managing complicated medical problems
Elite Care 2025 Fee Schedule:
$300 per month for individuals; or $3420 per year when paid annually
$275 each per month for multiples; $3135 each per year when paid annually
Custom Care Plans
We can customize plans with fewer guaranteed services at a discount. Custom care plans are negotiated on an individual basis. Call our practice administrator about customizing a plan to fit your needs and budget.
Outside Our Scope
Central Coast Direct is dedicated to providing comprehensive primary care medical services, but there are inherent limitations to examinations and procedures appropriate for a residential setting. We do not provide services normally performed in gynecology or urology medical offices, for example, or procedures better suited to an operative suite or surgery center. We do promise prompt referral for any needed services that we do not provide.
