Access Care
Access Care is designed for generally healthy adults who need routine medical care and reliable access to a provider. This plan includes guaranteed access to services, routine home visits, telemedicine care, and preventive health screenings.
Services include:
- One home visit per year
- Additional home visits if needed
- Telemedicine visits for urgent concerns
- Routine health screenings
Access Care – 2026 Fees
- $115 per month, or $1,380 per year when paid annually
- Household discounts do not apply to this plan.
Superior Care
Superior Care is recommended for patients with Medicare or chronic medical conditions who need additional monitoring and support. This plan includes all Access Care services and expanded care management.
Services include:
- Expedited consultations with a physician or nurse practitioner
- 2–4 home visits per year for chronic condition management
- Coordination with home health services, such as:
- Nursing care
- Physical therapy
- Occupational therapy
- Speech therapy
- Wound care
- Medical social services
- Timely referrals to specialists
- Review of specialist recommendations
- Cardiovascular risk assessments
- Annual wellness surveys
- Standard vaccine recommendations
- Periodic depression screening
- Prompt follow-up after hospital or emergency room visits
Superior Care – 2026 Fees
- Individual: $205 per month, or $2,337 per year when paid annually by check
- Multiple household members: $180 per person per month, or $2,052 per person per year when paid annually by check
Elite Care
Elite Care is designed for patients who need comprehensive in-home primary care with frequent monitoring and priority access to providers. This plan includes all Access Care and Superior Care services, as well as expanded clinical oversight and increased provider availability.
Services include:
- Priority scheduling for home visits
- Frequent home visits for ongoing care
- Unlimited visits for urgent medical concerns
- Extended time with your provider during visits
- Unlimited telemedicine consultations
- Disease prevention counseling
- Continuous chronic care management
- Immediate follow-up after hospital discharge or emergency care
- Advanced care planning for future medical needs
- Expedited specialist referrals, including university medical centers when needed
Elite Care – 2026 Fees
- Individual: $330 per month, or $3,762 per year when paid annually by check
- Multiple household members: $305 per person per month, or $3,477 per person per year when paid annually by check
Wellness and Weight Management
Our Wellness and Weight Management program is designed for patients using GLP-1 medications to support weight loss, type 2 diabetes, sleep apnea, and other metabolic health concerns. This membership provides personalized medical oversight, ongoing monitoring, and lifestyle guidance to help patients achieve sustainable, long-term health outcomes.
Services include:
- Management of GLP-1 medications by experienced medical providers
- Assistance with medication prior authorizations
- Prescription coordination through FDA-approved pharmacies
- We avoid using compounded GLP-1 medications to help minimize unnecessary fillers and additional ingredients
- Oral and injectable GLP-1 treatment options available
- Regular follow-up visits every 2–3 months
- Ongoing monitoring of treatment progress and response
- Nutrition, lifestyle, and wellness counseling
- Side effect management and medication adjustments as needed
- Personalized support throughout your health and weight management journey
- Comprehensive care focused on long-term metabolic health and wellness
Wellness and Weight Management – 2026 Fees
- Monthly Membership: $75 per month, including ongoing medication management and close clinical follow-up.
- Patients who would like primary care services in addition to GLP-1 management are encouraged to enroll in the Access Care plan. Please contact our practice administrator to discuss available options and determine the membership that best fits your healthcare needs.
